Assistance to Individual in Crisis Situation (AICS)

Medical Assistance

·         Any valid identification card of the client person to be interviewed

·         Medical Certificate or Medical Abstract with date of issuance, complete name. signature and license number of the attending physician (issued within three months), and

·          If payment for hospital bill, hospital bill statement, Statement of Account (Outstanding balance) with complete name and signature of the Billing Clerk, or

·         If for medicines/assistive devices - prescription with date of issuance, complete name, signature and license number of the attending physician; and certification from the Municipal Health Officer that the medicines are not, available in the Municipal Pharmacy; or

·         If for medical procedures - laboratory requests with date of issuance, complete name, signature and license number of the attending physician.

·         Barangay Clearance

·         Certificate of Indigency

Burial Assistance

·         Any valid identification card of the client/person to be interviewed

·          Death Certificate

·         Funeral Contract

·         Promissory note

·         Barangay Clearance

·         Certificate of Indigency

For transfer of cadaver:

·         Death Certificate

·          Transfer permit (if applicable)

Educational Assistance

·         Any valid identification card of the client/person to be interviewed

·         Certificate of Enrolment or Certificate of Registration

·         Current school ID

·         Statement of Account (if applicable)

·         Barangay Clearance

·         Certificate of Indigency
